Request CBS Facilities

If you plan to host an event at CBS or in association with CBS, you must fill out the following forms:

EVENT REQUEST FORM 
COMMUNICATIONS REQUEST FORM

***Please ensure these forms are completed AT LEAST two weeks prior to the event date/communication date. Any later and there is no guarantee CBS will be able to facilitate the event and share any necessary communications***
